Service Planning /Fetal Alcohol Spectrum Disorder (FASD) Coordinator

Reports to the Mental Health and Addictions Manager/ Child and Youth Mental Health Team Lead

Salary: $58,000

INTRODUCTION:

The Service Planning /FASD Coordinator will improve service experiences and outcomes for children and youth with multiple and/or complex special needs and their families in the Niagara Region. They will connect Child/Youth and their families to the multiple, cross-sectoral services they need as early as possible and monitor their needs and progress through a single coordinated service plan. Children/youth with a suspected or confirmed diagnosis of FASD can access an FASD Coordinator to assist with service coordination, and also to support service providers to take a supportive lens and consider the FASD-based needs in their approaches to programming and interventions. This position is overseen by De dwa da dehs nye>s Aboriginal Health Centre but the SPC/FASD Coordinator will work within a multi-disciplinary, collaborative team involving partners from a variety of sectors, including, but not limited to, developmental services, mental health, rehabilitation services, and Indigenous services, and other health and education sector partners. The position will provide support to the FASD Lead at Contact Niagara with the development, implementation, and delivery of various capacity building activities throughout the region (e.g., workshops, client and family groups, trainings, etc.), to continue to build capacity both directly with families, and across a multi-sectoral group of service partners across the region.

 

ROLES AND RESPONSIBILITIES:

The successful candidate will demonstrate exceptional ability to perform the following aspects of the position:

o   Provision of direct service coordination to families of Indigenous children and youth with complex special needs, which will include:

o   Providing case-specific service planning consultation/coordination/support to families of Indigenous children and youth with multiple and/or complex special needs by acting as the identifiable point of contact, and being responsible for coordinating/developing, with involved partners, a single coordinated service plan that recognizes all of their service needs and builds on their child/youth’s strengths;

o   Ensuring families connect with the right services, including culturally appropriate services, in their service delivery area, or elsewhere in Ontario as appropriate, and monitor how children and youth are progressing through the service plan as they grow;

Coordinating/facilitating/participating in cross-sectoral care-team meetings as needed to support development, consistent implementation, and ongoing review of the coordinated service plan.

o   Support the Aboriginal Health Centre and participate as a member of the Mental Health Team including:

o   Attending team and department meetings

o   Adhering to Health and Safety protocols

o   Participating in professional development activities and contributing to a culturally safe and positive work environment.

o   Supporting Contact Niagara as the CSP Lead as needed to ensure consistent and effective delivery of outcomes-based Coordinated Service Planning across service providers in Niagara, which will include:

o   Participating in implementation readiness and capacity building activities across the service systems in Niagara;

o   Supporting the development of planning processes towards consistent implementation and delivery;

o   Support Contact Niagara as the FASD Lead as needed to ensure consistent and effective delivery of FASD-based services and supports to providers and families/children/youth, which will include:

o   Participating in the development, design and delivery of FASD-focused supports and professional development activities across the service systems in Niagara;

o   Supporting the development and monitoring of Coordinated Service Plans for families of children/youth with diagnosed or suspected FASD;

o   Participating in the collaborative development and implementation of a program evaluation framework.

 

STATEMENT OF QUALIFICATIONS:

·         5+ years’ experience working with families, children, and youth with multiple and/or complex special needs, with demonstrated experience in coordinating services including navigating multiple service systems/sectors;

·         Demonstrated understanding of issues impacting families of children and youth with multiple and/or complex special needs and the exceptional provision of high quality, family-centered care within the context of recognizing the diversity of individual families;

·         Demonstrated experience in recognizing and understanding unique barriers to accessing services Indigenous people face, and experience in supporting this population in a culturally safe and competent manner

·         Bilingualism in French/English is considered an asset;

·         Demonstrated experience and competence in the practice of family/youth engagement at individual, organizational and system levels;

·         Demonstrated experience and competence working as part of a multi-disciplinary team, preferably working across agencies / systems in the provision of coordinated care;

·         Demonstrated experience in working with children/youth with diagnosed or suspected FASD and their families;

·         Ability to build and maintain effective, collaborative relationships with stakeholders;

·         Comprehensive knowledge base of children/youth and Indigenous services in the Niagara area, and demonstrated ability to access and work with these providers;

·         Knowledge of cultural traditions as well as, the history of colonization and intergenerational traumas impacting the health and wellness of Indigenous Peoples

·         Excellent interpersonal, communication, and organizational skills;

·         Problem solving, critical thinking, and conflict resolution skills;

·         Ability to work independently and collaboratively with interdisciplinary teams, families, and community services;

·         Demonstrated commitment to quality improvement and adherence to developed protocols;

·         Excellent time management and stress management skills;

·         Reliable Access to a vehicle, valid G license
